空中浩劫S01E01n波音在747上更换了货仓门闩的设计，本应切断的电力在关舱之后并没有发生，再加上钢换铝之后的强度不够导致舱门电路一旦发生短路，门就会自动打开。波音和FAA虽然已经在一次事故中发现了隐患但为了节约成本延长飞机更换舱门的期限。最终联合航空811次航班执飞檀香山到奥克兰航线终于在两万三千尺高度发生舱门撕裂，进而导致三号引擎全部四号引擎部分失效，虽然驾驶员凭借高超技术在超重超速情况下成功迫降，但机舱撕裂处五排座位九名乘客却被甩到飞机外丧生。nS01E02n1999年6月1日，受限于美联航派遣的压力，1420航班机长不得不在一片雷暴中执行航班任务。就在快要到达目的地机场时，受限于强风切变，机长决定反方向顺风着陆，机头的雷达便无法扫描雷暴云位置，再加上小机场雷达精密度不够，机长在一片狂风暴雨中根本看不到机场位置只能放弃目视采用无线电指引。种种操作令他焦头烂额却在着陆时忘了最基础的拉起扰流板，最终飞机着陆后无法减速冲出跑道装上天桥，机上十名乘客及机长本人丧命。nS01E03n1998年9月2日，当时最早一批头等舱拥有娱乐系统的MD11飞机瑞士航空111次从纽约飞往瑞士。起飞后驾驶舱空调背后线路起火开始冒烟，机长向空管发出PanPanPan信号告警并准备迫降。迫降预计25分钟同时进行故障检查20分钟，然后和目标机场高度过高距离过近再加上需要放油开始盘旋。然而火势越来越大，机上系统陆续失灵与空管失联后不久便坠海，甚至因为火势黑匣子都失效。调查人员大海捞针找出了产生电弧的电线，正是由于娱乐系统过热并且没有被汇流条开关正确关闭引起，再加上与驾驶舱的隔热隔音材料却是良好的助燃剂才引发了最终350G的坠海事故。nS01E04n秘鲁航空603次执飞波音757，驾驶员上飞机即立flag，飞机仪表比瑞士表还要精准，然后就是这个高度表和空速表起飞后便开始不动。随后虽然恢复了表面的正常，但是舱内频频出现相反的告警，例如超速与失速的警告同时响起，一边显示一万英尺的高度一边狂报近地撞击警告。最终驾驶员在一千犹豫操作中让飞机坠海，全部70人无人幸免。而这却只是因为地勤清洗时在静压孔上贴上的胶布忘了取下，从而导致了飞行员在夜间失去目视之后又丧失了仪表辅助驾驶的能力。nS01E05n2000年1月31日，美国阿拉斯加航空261号执飞MD83飞机起飞后水平安定面卡死，飞机开始低头，驾驶员推测驱动马达故障尝试重启失败后，关闭自动驾驶大力拉杆爬升，飞机在经历连续俯冲之后勉强被控制住。随后安定面突然彻底甩开失效，飞机从一万八千英尺开始垂直俯冲直至坠海，88名乘客和机组人员无一幸免。nMD83飞机采用t形尾翼+全动平尾的设计，调查发现由于驱动平尾运动的伸缩螺杆机构内部的螺纹被磨平而导致了平尾的卡死，驾驶员几次的尝试驱动最终导致螺帽脱落安定面甩开，而最初的滑丝仅仅因为航司为了减少开销延长两次维护时间间隔最终导致螺杆螺帽间失去润滑能力。当然麦道公司仅靠一个螺杆螺帽来支撑水平安定面而没有备用冗余也是重要的设计缺陷。nS01E06n驾驶员究竟是英雄还是罪魁祸首？2001年8月24日，越洋航空236号航班空客A320驾驶员因为右发漏油燃料耗尽而打开油料交换开关，尽管在此之前计算机一直报告油温过低油压过高之后也不断发出燃料即将耗尽的警告，但他扔固执认为must be the computer是计算机的故障而没有关掉交换开关，最终离在机场157km的高空两台发动机全部停车开始滑翔。好在冲压空气涡轮机启动发电为液压提供动力，驾驶员此时墨镜一戴谁也不爱开始展现真正的技术尽管以高速砸地爆了八个轮胎但仍然安全停了下来。n事后发现五天前的一次地面维修，明明发动机配套液压管为老款，航司仍然逼迫地勤坑机组将它换上，导致飞行过程中与输油管不断摩擦最终磨破了输油管发生了泄油事故。而驾驶员的处理究竟是直接凶手或是英雄机长却从此没有了处理结果。
